You might not expect a men’s health clinic in Western NY that launched mid-pandemic to become a runaway success, but that appears to exactly have been what has happened in the case of Adult Health NP of WNY in Rochester, New York.
The clinic opened in January 2022 while the COVID-19 pandemic was ongoing. Thanks in part to satisfied clients spreading word of mouth, Adult Health NP of WNY is already one of Western New York’s largest men’s health clinics. Its practice attempts to bring a more modern approach to men’s health with testosterone replacement therapy in Rochester, NY (TRT), medical weight loss treatments, and peptide therapy.
The clinic also seeks to adopt a more patient-first approach to delivering health services, replacing conventional medicine’s often cold and impersonal approach with longer appointments, faster turnaround times, and direct patient-provider communication.
Igor Gorchakov, who founded the clinic, moved to the United States with his family from Ukraine as a child. “We were a poor, newly immigrated family trying to find our footing,” he says. “We relied on the local church for a lot of basic things. I remember during Christmas, they let us submit a wish list. I wanted a remote control car—but my mom asked for blankets and bed sheets instead. What kid wants blankets?”
Like many immigrant parents, his mom and dad believed education was the path to a better future. At first, Gorchakov was hesitant about nursing school because it was seen as a feminine career. “My dad told me to just go for it. If I didn’t like it, I could switch. But at least I’d be moving toward something.”
He became a registered nurse, then a nurse practitioner. Along the way, he noticed how broken parts of the medical system were—long wait times, rushed visits, and a heavy push toward medications, especially for men in their thirties and forties dealing with fatigue and weight gain.
Looking for a different way to connect, he started a YouTube channel. “It was just for fun—I’d post videos with my Rottweiler, Bruno, while I was in school. Somehow we built a following.” That channel grew to over 100,000 subscribers, but with three kids and a growing business, he eventually had to step back from posting.
Then, he opened Adult Health NP of WNY to provide higher-quality care and access to a broader range of treatments, particularly for hormone optimization in NY and weight loss. Gorchakov wanted to build a practice where men could feel heard, spend time planning real solutions with their medical providers, and have more direct access to their providers.
Opening in the middle of COVID-19 was risky, and Gorchakov also had to build the business from scratch without corporate backing—just the reach of his YouTube channel and the goodwill of satisfied patients who could spread the word.
And there were many. One says, “UPDATE: Been on TRT for almost 2 years now. I have lost 65 Ibs and feel 15 years younger. I am 53 years old and started to feel like it. After comprehensive blood testing and in-person consultation, Igor started me on TRT. Going forward, Igor has been great at answering questions and addressing any concerns. I am so pleased with the treatment and results that I’ve convinced my 50-year-old brother to see Igor.”
His clinic also had to overcome skepticism and pushback against alternative treatments like TRT. Nevertheless, the Adult Health NP of WNY has scaled to meet a growing demand to such a degree that Gorchakov now aspires to expand the clinic across New York, opening more locations and hiring more nurse practitioners.
Gorchakov also hopes to add women’s hormone optimization to the services. As part of its next phase, the clinic has partnered with the marketing and development firm Just Get Known to expand its reach across New York and into neighboring states. Just Get Known recently redesigned the clinic’s website to better serve a growing regional audience and support future growth beyond state lines.
For now, however, the clinic is focused on men’s health, and its success is built on Gorchakov’s commitment to personalized care. Patients are seen in days, not weeks or months, and the average appointment lasts an hour, making it possible for health providers to have in-depth conversations with patients and plan more thorough care. Patients can call or text their providers directly.
Gorchakov remarks that the industry makes healthcare seem complicated, but “the truth is simple: care about your patients, and they’ll come back.” For Adult Health NP of WNY, caring about patients means rejecting the conventional approach of rushed, impersonal care and providing men’s health alternatives—like testosterone optimization and medical weight loss treatments in Rochester—that the clinic’s team believes can be life-changing.
